This Mailbox Service Agreement ("Agreement") is made and entered into by the customer identified above ("Customer") for the use of and services related to a mailbox (the "mailbox") at EASTERN SHORE SHIP PRINT CONNECT doing business as EASTERN SHORE SHIP PRINT CONNECT, hereinafter referred to as (“EASTERN SHORE SHIP PRINT CONNECT”) under the terms set forth herein and governed by the laws of the State of Maryland.

1. Customer agrees that the customer will not use EASTERN SHORE SHIP PRINT CONNECT premises or any EASTERN SHORE SHIP PRINT CONNECT services for any unlawful, illegitimate, or fraudulent purpose or for any purpose prohibited by U.S. Postal regulations. The customer further agrees that any use of the mailbox shall be in conformity with all applicable federal, Republic, and local laws. Each individual or entity must complete a separate U.S. Postal Service Form 1583 ("Form 1583") to be authorized to receive mail or packages at the mailbox. However, spouses may complete one Form 1583, as long as both spouses include their separate information on the Form.

2. This agreement and Form 1583 shall remain confidential, except that this agreement and Form 1583 may be disclosed upon written request of any law enforcement or other governmental agency, or when legally mandated. Upon request, the customer agrees to complete all necessary documents, including Form 1583 and any required acknowledgment form relating to the service of the process. The customer further agrees to sign an updated version of this agreement and Form 1583 upon request.

3. Possession of the mailbox username and password shall be considered valid evidence that the possessor is duly authorized to remove any contents from the mailbox. In the event of death or incapacity of the customer, EASTERN SHORE SHIP PRINT CONNECT will require the appropriate documents from the Probate Court, the executor of the estate, the trustee, or other similar person or entity before releasing mail or packages to a requesting party.

4. Upon expiration, cancellation, or termination of this agreement, EASTERN SHORE SHIP PRINT CONNECT will:

a.Re-mail (i.e., forward) customer's mail for six (6) months, provided the customer pays the postage, packaging material, and forwarding fees in advance. Additionally, the customer must pay a monthly storage fee of $10.00 for month 1, and to be determined for months 2 through 6 in advance for the time period that mail is to be forwarded. It is the customer's responsibility to make arrangements with EASTERN SHORE SHIP PRINT CONNECT to identify any mail forwarding needs prior to the expiration, cancellation, or termination of this agreement.

b. Store the mail or packages for up to six (6) months provided the customer pays a storage fee of $20.00 per month for the time period in which EASTERN SHORE SHIP PRINT CONNECT holds the mail or package(s), plus a service fee of to be determined for each time customer visits EASTERN SHORE SHIP PRINT CONNECT to pick up such items. It is the customer's responsibility to make arrangements with EASTERN SHORE SHIP PRINT CONNECT to identify any mail storage needs prior to the expiration, cancellation, or termination of this agreement.

c. Retain customer's mail, other than Unsolicited Mail, at the Center for a period of ten (10) days, if the customer leaves no forwarding fees and forwarding address. After such time, any mail or package may be discarded or destroyed.

d. Discard or destroy any "Unsolicited Mail" (e.g., bulk mail; mail addressed as "occupant," "current resident" or similar designation; or coupons, advertising, or other promotional material) delivered to or remaining at EASTERN SHORE SHIP PRINT CONNECT.

e. Refuse any package addressed to the customer delivered by any party other than the U.S. Postal Service, such as a commercial courier service.

5. Upon cancellation or termination of this agreement, EASTERN SHORE SHIP PRINT CONNECT may:

a. Refuse any mail or package addressed to the customer and delivered to EASTERN SHORE SHIP PRINT CONNECT.

b. Discard or destroy any of the customer's mail or packages delivered to or remaining at EASTERN SHORE SHIP PRINT CONNECT at such time.

6. The term of this agreement shall be the initial period paid for by the customer and any renewal period paid for by the customer from time to time. Renewal of this agreement for additional terms shall be at EASTERN SHORE SHIP PRINT CONNECT’s sole discretion.

7. Customer agrees that the Center may terminate or cancel this agreement for good cause at any time by providing the Customer thirty (30) days written notice. Good cause shall include, but is not limited to 1) Customer abandons the mailbox; 2) Customer uses the mailbox for unlawful, illegitimate or fraudulent purposes; 3) Customer fails to pay monies owed when due; 4) Customer receives an unreasonable volume of mail or packages; 5) Customer engages in offensive, abusive or disruptive behavior toward other customers of EASTERN SHORE SHIP PRINT CONNECT or EASTERN SHORE SHIP PRINT CONNECT's employees; and 6) Customer violates any provision of this agreement. The customer acknowledges that, for the purpose of determining good cause for termination of this agreement as provided herein, the actions of any person authorized by the customer to use the mailbox will be attributed to the customer.

8. Any written notice to the customer required or permitted under this agreement shall be deemed delivered twenty-four (24) hours after the placement of such notice in the customer's mailbox or at the time personally delivered to the customer. In the event of a termination notice, based upon abandonment of the mailbox, notice shall be deemed delivered (a) on the next day after placing in the hands of a commercial courier service or the United States Postal Service for next day delivery, or (b) five (5) days after placement in the United States Mail by Certified Mail, Return Receipt Requested, postage prepaid, and addressed to customer at customer's address as set forth in Form 1583, or on the date of actual receipt, whichever is earlier.

9. As the customer's authorized agent for receipt of mail, EASTERN SHORE SHIP PRINT CONNECT will accept all mail, including registered, insured, and certified items. Unless prior arrangements have been made, EASTERN SHORE SHIP PRINT CONNECT shall only be obligated to accept mail or packages delivered by commercial courier services that require a signature from EASTERN SHORE SHIP PRINT CONNECT as a condition of delivery. Customers must accept and sign for all mail and packages upon the request of EASTERN SHORE SHIP PRINT CONNECT. Packages not picked up within three days of notification will be subject to a storage fee of $5.00 per day per package, which must be paid before the customer receives the package. In the event a customer refuses to accept any mail or package, EASTERN SHORE SHIP PRINT CONNECT may return the mail or package to the sender and the customer will be responsible for any postage or other fees associated with such return. C.O.D. items will be accepted ONLY if prior arrangements have been made and payment in advance is provided to EASTERN SHORE SHIP PRINT CONNECT.

10. Customer agrees to protect, indemnify, defend and hold harmless EASTERN SHORE SHIP PRINT CONNECT, and their respective affiliates, subsidiaries, parent corporations, franchisees, officers, directors, agents, and employees from and against any and all losses, damages, expenses, claims, demands, liabilities, judgments, settlement amounts, costs and causes of action of every type and character arising out of or in connection with the use or possession of the mailbox, including without limitation, any demands, claims, and causes of action for personal injury or property damage arising from such use or possession, from the failure of the U.S. Postal Service or any commercial courier service to deliver on time or otherwise deliver any items (mail, packages, etc.), from damage to or loss of any package or mail, or to the mailbox contents by any cause whatsoever, and from any violation by customer of applicable federal, Republic or local laws.

11. Customer HEREIN AGREES THAT THE TOTAL AMOUNT OF LIABILITY OF EASTERN SHORE SHIP PRINT CONNECT, IF ANY, FOR ANY AND ALL CLAIMS ARISING OUT OF OR RELATED TO THIS AGREEMENT SHALL NOT EXCEED $100.00 REGARDLESS OF THE NATURE OF THE CLAIM.

12. Customers must use the exact mailing address for the mailbox without modification as set forth in Section three (3) of Form 1583. The Postal Service will return mail without a proper address to the sender endorsed "Undeliverable as Addressed."

13. Delivery by commercial courier services must be made to EASTERN SHORE SHIP PRINT CONNECT street address only (and not to a P.O. Box). "P.O. Box" may be used only if it is part of the customer's "Caller Service" (arrangement for delivery of mail through EASTERN SHORE SHIP PRINT CONNECT using a U.S. Postal Service address) address format. Upon signing this agreement, the customer shall provide two forms of valid identification, one of which shall include a photograph. This agreement may not be amended or modified, except in a writing signed by both parties.

14. EASTERN SHORE SHIP PRINT CONNECT reserves the right to open and inspect any packages or mail which may arrive damaged or that may be otherwise of questionable integrity or legality.

15. By agreeing below I forfeit all right to bring a suit against EASTERN SHORE SHIP PRINT CONNECT for any reason. In return, I will receive the mailbox and related services. I will also make every effort to obey U.S. law and the laws of Maryland, as listed in writing and as explained to me verbally. I will ask for clarification when needed.

16. The customer shall indemnify and hold harmless the Company, its affiliates, and its respective officers, directors, agents, and employees from any and all claims, demands, losses, causes of action, damage, lawsuits, judgments, including attorneys’ fees and costs, arising out of or relating to, the EASTERN SHORE SHIP PRINT CONNECT services under this agreement.

17. This agreement, and any accompanying appendices, duplicates, or copies, constitute the entire agreement between the Parties with respect to the subject matter of this agreement and supersedes all prior negotiations, agreements, representations, and understandings of any kind, whether written or oral, between the Parties, preceding the date of this agreement.

18. This agreement may be amended only by a written agreement duly executed by an authorized representative of each party (email is acceptable).

19. If any provision or provisions of this agreement shall be held unenforceable for any reason, then such provision shall be modified to reflect the parties’ intention. All remaining provisions of this agreement shall remain in full force and effect for the duration of this agreement.

20. This agreement shall not be assigned by either party without the express consent of the other party.

21. A failure or delay in exercising any right, power, or privilege in respect of this agreement will not be presumed to operate as a waiver, and a single or partial exercise of any right, power, or privilege will not be presumed to preclude any subsequent or further exercise, of that right, power or privilege or the exercise of any other right, power or privilege.

22. This agreement is governed by and construed in accordance with the laws of Maryland without reference to any principles of conflicts of laws, which might cause the application of the laws of another Republic Admitted to the Union (the “United States of America”, or the “Union”). Territories and Protectorates of the Union are hereby discarded and no action may be taken within them against EASTERN SHORE SHIP PRINT CONNECT

23. The laws of Maryland shall supersede the United Nations Convention on Contracts for the International Sale of Goods (CISG) and all other United Nations laws.

24. Any action instituted by either party arising out of this agreement will only be brought, tried, and resolved in the applicable federal or Republic Courts having jurisdiction in Maryland. EACH PARTY HEREBY CONSENTS TO THE EXCLUSIVE PERSONAL JURISDICTION AND VENUE OF COURTS HAVING JURISDICTION IN THE STATE

Addendum additional agreement for FEDEX shipments-
You are prohibited from tendering the following items for shipment, and you agree not to do so:
a. Cash and currency.
b. Live animals, except as provided in the Animals, Ornamental Marine Life (Including Live
Fish), and
Animal Carcasses section. (Edible seafood, such as live lobsters, crabs, or other types of fish and
shellfish
for human consumption, is acceptable, provided the shipper is in compliance with all local, state,
and
federal laws.)
c. Animal carcasses, except as provided in the Animals, Ornamental Marine Life (Including Live
Fish),
and Animal Carcasses section. (This restriction does not apply to properly packaged meat or
poultry
products intended for human consumption.)
d. Human corpses, human organs or body parts, human embryos, or cremated or disinterred
human
remains.
e. Shipments that require us to obtain a local, state, or federal license for their transportation.
f. Shipments that may cause damage or delay to equipment, personnel, or other shipments.
g. Items resembling a bomb, hand grenade, or other explosive device, except as provided in the
Dangerous Goods section. This includes, but is not limited to, inert products such as novelty
items,
training aids, and works of art.
h. Bump fire stocks and other rapid-fire trigger activators.
i. Ghost guns and other firearms, frames, or receivers not containing a serial number, including,
but not
limited to, items that are designed to or may readily be completed, assembled, restored, or
otherwise
converted to function as a frame or receiver, as defined in 27 CFR 478.12.
j. Any firearm or other weapon manufactured using a 3-D printing machine.
k. 3-D printing machines designed, or that function exclusively, to manufacture firearms.
l. Lottery tickets and gambling devices where prohibited by law.
m. Hazardous waste. This includes, but is not limited to, used hypodermic needles or syringes
transported for sterilization, recycling, disposal, or for any other purpose, or other medical waste.
n. Packages that are wet, leaking, or emit an odor of any kind.
o. Live insects.
p. Shipments or commodities that are prohibited by applicable local, state, or federal law.
q. Counterfeit goods, including, but not limited to, goods under a trademark that is identical to or
substantially indistinguishable from a registered trademark, without the approval or oversight of
the
registered trademark owner (also commonly referred to as “fake goods” or “knock-offs”).
r. Tobacco and tobacco products, including but not limited to cigarettes, cigars, loose tobacco,
smokeless tobacco, hookah, or shisha.
s. Electronic cigarettes and their component parts, any other similar device that relies on
vaporization
or aerosolization, and any noncombustible liquid or gel, regardless of the presence of nicotine,
that can
be used with any such device. (Effective March 1, 2021)
t. Marijuana, as defined by U.S. federal law, 21 U.S.C. 802(16), including marijuana intended for
recreational or medicinal use and marijuana-derived cannabidiol (“CBD”); any product with a
delta-9-
tetrahydrocannabinol (“THC”) concentration of more than 0.3% on a dry weight basis, except as
set out
in 21 CFR 1308.35; and synthetic cannabinoids.
u. Raw or unrefined hemp plants, or their subparts (including, but not limited to, hemp stalks,
hemp
leaves, hemp flowers, and hemp seeds), except as set out in 21 CFR 1308.35.
v. Any substance that has not been approved for a medical use by the U.S. Food and Drug
Administration and also has been listed as a Drug or Chemical of Concern by the U.S. Drug
Enforcement
Administration, including, but not limited to, kratom and Salvia divinorum.
w. Waste or garbage for disposal.
x. These additional items are prohibited by FedEx SameDay and FedEx SameDay Freight: –
Alcoholic
beverages. – Dangerous goods, including Common Fireworks (Division 1.4 explosives). The
following
dangerous goods are accepted for shipment: dry ice and Biological Substance, Category B (UN
3373). –
Firearms. – Live animals, regardless of any approval as provided in the Animals, Ornamental
Marine Life
(Including Live Fish), and Animal Carcasses section. Edible seafood is accepted; see (b) above.
– Plants
and plant materials. Notwithstanding any other provision of the FedEx Service Guide, we are not
liable
for delay of, loss of, or damage to a shipment of any prohibited item. The shipper agrees to
indemnify
FedEx for any and all costs, fees, and expenses FedEx incurs as a result of the shipper’s violation
of any
local, state, or federal laws or regulations or from tendering any prohibited item for shipment

Addendum Additonal Agreements for UPS Shipments:
You are prohibited from shipping and agree not to ship or tender for shipment the following items:
Alcoholic Beverages Packages containing alcoholic beverages
(beer, wine or spirits) are accepted for transportation only on a contractual basis. Shippers must be
licensed and authorized under applicable law to ship alcoholic beverages. Alcoholic beverages are
accepted fort transportation only to certain states. To receive service for packages contain - ing alcoholic beverages,the shipper must enter into, and agree to, the provisions set forth in the approved UPS agreement for the transportation of alcoholic beverages shipments. Shipments containing alcoholic beverages can be delivered to an alternate address via UPS Delivery Intercept® or redirected to a UPS Access Point® Location via UPS My Choice.® C.O.D. Collect on Delivery (C.O.D.) service is not available for international services except for U.S. imports from Canada. C.O.D. service is not available for UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight services. C.O.D. service is also not available when using a UPS 3 Day Select® or a UPS® Ground shipping document. Firearms, Ammunition and Weapons Firearms, ammunition and weapons (including inert and replica explosive weapons) will be accepted for transportation only according to the “UPS Tariff/Terms and Conditions of Service – United States” available at ups.com/ termsofservice. Jewelry, Unset Precious Stones and Industrial Diamonds International jewelry shipments, and shipments of unset precious stones or industrial diamonds, are subject to maximum declared value restrictions that vary by destination as set forth at ups.com/jewelry. Maximum Length The maximum length per domestic, export or import package is 108 inches. Maximum Size The maximum size per domestic, export or import package is 165 inches in length plus girth [(2 x
width) + (2 x height)] combined. The maximum size per UPS® Simple Rate package is 1,728
cubic inches. For irregularly shaped packages, refer to page 19 for instructions. UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight pallets are subject to maximum size restrictions (which vary by origin and destination) as set forth at ups.com/wwefmaxdimweight. Prior approval is required to ship UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight shipments exceeding maximum size or weight restrictions. Maximum Declared Values Shippers are prohibited from shipping, and UPS does not accept for transportation, packages having a value of more than $50,000.00,except for a package eligible for the Enhanced Maximum Declared Value of $70,000.00 per package, as set forth in the “UPS Tariff/Terms and Conditions of Service – United States” available at ups.com/termsofservice. A package is eligible only if it is a domestic UPS Next Day Air® delivery service shipment, is tendered pursuant to shipper’s scheduled pickup service, is processed for shipment using a UPS Shipping System (declarations of value on paper source documents are not eligible) and does not contain hazardous materials or a perishable commodity. For UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight services, UPS does not accept for transportation pallets having
a value of more than $100,000.00 per pallet. Additionally, more restrictive value limits apply for certain packages and pallets. Refer to the “UPS Tariff/Terms and Conditions of Service – United States” available at ups.com/termsofservice for more information on maximum allowable values and liability limitations. Maximum Weight For domestic package shipments, the maximum actual weight per package is 150 pounds. The maximum actual weight per UPS Simple Rate package is 50 pounds. For international shipments, the maximum weight of a package may vary by destination or origin. UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight pallets are subject to maximum weight restrictions (which vary by origin and destination) as set forth at ups.com/wwefmaxdimweight. Prior approval is required to ship UPS Worldwide Express Freight Midday and UPS Worldwide Express Freight shipments exceeding maximum size or weight restrictions. Maximum Weight for a Hazardous Materials and International Dangerous Goods Package The maximum weight for a Hazardous Materials and International Dangerous
Goods package is 70 pounds, unless further restricted by the regulations set forth in 49 CFR or IATA
regulations. Maximum weight for a non-Hazardous Materials package shipment containing dry ice is 150 pounds. Consult the “UPS Guide for Shipping Ground and Air Hazardous Materials” at ups.com/ hazmat or call the Hazardous Materials Support Center at 1-800-554-9964 for more information. Restricted and Prohibited Articles (Export and Import) It is the shipper’s responsibility to comply with current government regulations or laws applicable in each country. Shipments are subject to inspection and possible delay by customs officials or representatives of other government agencies. Certain countries and territories have additional prohibited or restricted articles. For more information, call the UPS International Customer Service Center at 1-800-782-7892. The following items are restricted for international shipping. They may be shipped into the U.S., and to countries and territories outside the U.S., only where service is allowed on a contractual basis: – Alcoholic beverages – Animal products, nondomesticated (e.g., mother of pearl inlay, snakeskin watch bands) – Articles of high/unusual value –
Biological substances, Category B and exempt human or animal specimens – Dangerous goods –

The following items are not accepted for transportation by UPS into the U.S. or to

Shipments prohibited by law Tobacco and Vaping Products Shipments containing tobacco
products
(“Tobacco Product Shipments”), are accepted for transportation only from shippers who are
licensed
and authorized to ship tobacco products pursuant to applicable laws. UPS does not provide
service to
any person or entity listed in the Bureau of Alcohol, Tobacco, Firearms and Explosives PACT
Act – Non-
Compliant List. The term “Tobacco Products” is defined at ups.com/tobacco. Shippers should
consult
ups.com/tobacco for the current definition of tobacco products, as the definition may change
periodically. At present, and without limiting the complete definition posted at ups.com/tobacco,
the term “Tobacco Products” includes any product made or derived from tobacco that is intended for
human consumption, including any component, part or accessory of a tobacco product; the term
includes, without limitation, e-cigarettes, e-hookah, e-cigars, vape pens, advanced refillable
personal vaporizers and electronic pipes. UPS prohibits the shipment of Cigarettes or Little Cigars (as those terms are defined at ups.com/tobacco) to consumers. UPS also prohibits the shipment of all Vaping Products (as the term is defined at ups.com/tobacco) throughout its U.S. domestic network, including import and export, regardless of nicotine content and destination state. To make other Tobacco Product Shipments, the shipper must sign, agree to and comply with the provisions set forth in an approved UPS agreement for the transportation of Tobacco Products. Shippers and receivers must comply with all applicable federal, state, provincial or local laws or regulations, and all Tobacco Product Shipments must conform to the terms, conditions, restrictions and prohibitions set forth at ups.com/tobacco at the time of shipping. It is the responsibility of the shipper to ensure that a shipment tendered to UPS, including a Tobacco Product Shipment, does not violate any federal, state, provincial or local laws or regulations applicable to the shipment. All shipments, including shipments containing Tobacco Products, will be accepted for transportation only according to the “UPS Tariff/Terms and Conditions of Service – United States” available at ups.com/termsofservice. Tobacco Products may not be shipped from a UPS Drop Box, UPS Customer Center, UPS Alliance Location (Staples), locations of The UPS Store® , any UPS Authorized Shipping Outlet or UPS Access Point® Location. Please visit ups.com/tobacco for more information. UPS Returns® UPS Returns Services are not provided via UPS Next Day Air Saver® or UPS 2nd Day Air A.M.® service levels. UPS Returns Service options for UPS® Simple Rate are limited to UPS Print Return Label Service only. Hazardous Materials or Dangerous Goods packages, pallets requiring shipping papers and packages containing firearms cannot be retrieved via UPS Returns Services. Biological substances and dry ice can be retrieved via UPS Returns Services. No other restricted or prohibited articles may be retrieved with UPS Returns Services. Refer to ups.com/termsofservice for value limits on UPS Returns Services shipments. Multiple-piece shipments are not available for UPS Returns Services shipped within the 50 states and Puerto Rico
